The attached application form below is to be completed, signed, and emailed to network@momentum.co.za

Once the application is returned, it will be assessed, and feedback will be provided, on whether the application was successful or declined.

Should the application be successful, the agreements will be emailed to the practice including the rates.

The banking details process is as follows:

  • A certified copy of ID for all doctors in the practice
  • An original canceled cheque, or
  • An original letter from the bank confirming the bank details, or
  • A certified copy of the bank statement (not older than 3 months)
  • The email should be sent from the registered email address we have on system and emailed directly to  providerbankingdetails@momentum.co.za.
