We live in challenging times. The increased demand for value-based, person-centred care has gained momentum at precisely the same time as healthcare costs continue to soar, leaving us with little to no choice but to disrupt age-old models, and shift the conversation from treating to preventing; from prescribing to promoting wellness; from hospital treatment to encouraging home-based care; and from physician focus to patient empowerment.
Dr Riaz Motara reminds us that as mobile devices continue to transform almost every facet of our daily lives, connected health solutions and social media platforms have changed the healthcare landscape and provide new opportunities to connect patients, providers and payers. “Clinicians must catch up and be part of this change. Empowered, engaged e-patients are growing a social movement and spearheading the shift,” says Dr Riaz Motara.
It is now time for a new approach to the entire continuum of care, from screening and diagnosis to standardised management, to promoting adherence and compliance, home monitoring and patient engagement. An ecosystem that connects, and creates an information flow between various providers to deliver personalised, predictive, precise, and preventative patient-centred care with and value-based outcomes.
